WAIVER AND RELEASE OF LIABILITY Short-Term Child Supervision
I, the undersigned, understand and acknowledge that Tribe Fitness is not a licensed childcare facility, and that the temporary supervision of my child/children is a service Tribe may provide only while I am present at the training site (“Location”) for the sole purpose of participating in Tribe Fitness training or classes (“Training”).
I hereby grant permission for my child to be temporarily supervised by Tribe Fitness (including it’s officers, employees, representatives, agents, contractors, volunteers, successors and assigns - collectively, "Tribe") while I participate Training. In the event that I am injured during Training and that injury makes me unavailable or unable to attend to my child, I hereby give Tribe my express written permission to contact the Emergency Contact listed herein, and acknowledge that they may release my child into their custody if required.
I understand that Tribe reserves the right to decline temporary supervision of any child for any reason, at Tribe’s sole discretion. I hereby hold harmless, waive and release Tribe from any and all liability in connection with any personal injury or property damage or loss that may occur while my child is supervised by Tribe at any Location. I agree not to sue Tribe, and release all rights to make a claim agains or sue Tribe in connection with the supervision of my child . I further agree to defend and indemnify the Tribe for any and all claims arising in connection with Tribe’s supervision of my child.
I understand that if my child should become inconsolable or present any behavioral issues negatively impacting other children or Tribe staff during the training session, it is my responsibility to leave Training and attend my child. I understand it is my sole responsibility to inform Tribe with any and all information about any allergies, sensitivies, behavioral issues my child may have that may impact my child or any other child’s health or well-being.
SICK CHILD POLICY
It’s simple. If your child has:
1) A fever or has had a fever within the last 24 hours
2) Mucus that is green, white, or brown
3) A persistent hacking cough
4) Unexplained rash or blisters
7) Pink Eye
PLEASE keep him/her home until symptoms have been absent for 24 hours without medication. Every Tribe participant should feel comfortable utilizing the childcare service without worry her child will become ill. We have young babies, toddlers, and older children in the same area multiple days per week and want to do our best to avoid spreading illness in general, but especially to younger, more vulnerable attendees. If a child is a bit “under the weather," he or she is welcome to stay with the parent for the workout, granted the parent assumes responsibility for keeping child separated from the other children for the duration of class.
I, the undersigned, have read and understand the Tribe Fitness Sick Child Policy and will make every effort to abide by its guidelines as part of a collective effort to keep all parties (group fitness participants, childcare attendees, and childcare staff) free from illness. I understand that if my child is exhibiting any of the symptoms of illness listed above, I may be asked to remove my child from the childcare and/or group workout site and return when symptoms have been absent for 24 hours without medication.
